This video appointment is for Veterans requesting a comprehensive mental health evaluation and consideration for completion of one Department of Veterans Affairs Disability Benefits Questionnaire (DBQ). The appointment includes a face-to-face video clinical interview, mental status examination, assessment of current symptoms and functional limitations, review of relevant medical documentation, and completion of one mental health DBQ when clinically appropriate and supported by the evaluation and available records. This service may be appropriate for mental health conditions such as PTSD, depression, anxiety, bipolar disorder, and other psychiatric conditions that are within the provider’s professional scope of practice. WHAT IS INCLUDED • One 90-minute video clinical evaluation • Review of one current mental health DBQ • Review of up to 50 pages of directly relevant records • Assessment of symptoms, treatment history, social functioning, and occupational impairment • Completion and signature of one DBQ when clinically supported • A copy of the completed DBQ for the Veteran’s records and submission to the VA REQUIRED DOCUMENTS Please upload the following through the secure patient portal at least five business days before your appointment: • The current VA DBQ you are requesting • Relevant military, VA, and private treatment records • Prior Compensation and Pension examination reports, if available • Psychological testing or hospitalization records, if applicable • A current medication list • A brief timeline describing the onset, treatment, progression, and functional impact of your symptoms Please email our office your government-issued photo ID to the appointment. Do not enter or email your full Social Security number or VA claim number through the online booking form. IMPORTANT INFORMATION This is not a drop-off paperwork service. The provider must independently evaluate the Veteran and review the available clinical evidence before determining whether the requested DBQ can be completed. Payment does not guarantee that a DBQ will be completed, accepted by the VA, or result in approval of benefits, a favorable decision, or a specific disability rating. The provider may decline to complete the DBQ if the request is outside the provider’s scope of practice, the records are insufficient, the requested conclusions are not clinically supported, or an appropriate evaluation cannot be completed.
