The Comprehensive Personal Injury Evaluation offers an in depth psychiatric assessment for individuals experiencing significant psychological or functional changes after an accident or injury. This service provides expanded documentation review, advanced diagnostic clarification, and a detailed narrative report designed to support complex clinical, legal, and insurance needs. Evaluation Components Two Hour Comprehensive Psychiatric Interview Assesses • Emotional and behavioral response to the incident • Trauma related symptoms including reactivity, avoidance, and hypervigilance • Anxiety, panic, stress intolerance, and physiological arousal • Mood disturbance, irritability, and difficulty regulating emotions • Sleep disruption, nightmares, or fatigue • Cognitive concerns including memory, concentration, and processing speed • Impact on work, school responsibilities, interpersonal functioning, and daily activities • Psychiatric and medical history • Pre existing conditions and contributing risk factors • Current treatment involvement, coping strategies, and recovery barriers Extensive Record Review Up to 100 Pages Includes • Medical and hospital records from multiple providers • Therapy and mental health documentation • Accident reports and police summaries • Imaging results and specialty assessments • Employer or school documentation reflecting functional changes • Photographs or injury related evidence • Attorney submitted materials establishing chronology This expanded review supports a well substantiated and clinically sound formulation. Clinical Screening and Assessment Tools Used as appropriate • Trauma symptom scales • Depression and anxiety measures • Functional impairment tools • Cognitive and attention screening • Pain related psychological assessments when relevant Detailed Diagnostic Formulation Identifies • Injury related psychiatric disorders • Worsened or newly emerging symptoms • Symptom severity, chronicity, and functional limitations • Clinical relationship between the incident and current presentation • Factors influencing prognosis and treatment needs Comprehensive Narrative Report Includes • Purpose of evaluation • Symptom history and timeline • Record review findings • Mental status examination • Screening results • Diagnostic impressions with rationale • Causation opinion when appropriate • Functional impact summary • Recommended treatment and follow up care Includes attorney clarity on findings, questions, and next steps.

Refund & Cancellation Policy Atlanta Psych and Wellness Center is committed to compassionate care and fair, transparent billing. 1. Prepaid Appointments Some services (like new patient visits) require full prepayment to confirm your appointment. This amount goes toward your visit balance. 2. Cancellations & No-Shows 24-hour notice is required to cancel or reschedule. Late cancellations or no-shows will be charged 50% of the visit fee. This applies to all appointments, including technical issues caused by the patient (e.g., late login). Note: These fees are not billable to insurance and must be paid before future bookings. 3. Refunds Services that have already been provided are non-refundable. Approved refunds will exclude processing fees, which are charged by our payment processor and not retained by our office. To request a refund, email: atlantapsychllc@gmail.com with your full name, date of service, and reason. Requests are reviewed within 5 business days. 4. Chargebacks Filing a chargeback instead of contacting us directly causes unnecessary costs to the practice. Policy: Accounts with chargebacks will be permanently closed. No future appointments can be booked on that account. Please reach out to resolve billing concerns professionally. We understand emergencies happen, please contact us if you need help. Repeated late cancellations or no-shows may result in discharge from the practice.
