Service Description

We provide comprehensive psychiatric evaluations and detailed medical documentation to support Social Security Disability (SSI/SSDI) claims. This service is designed for individuals who require clear, clinically supported evidence of mental health conditions and functional limitations. Each evaluation includes a thorough clinical interview, detailed review of psychiatric history, and completion of required Social Security forms with supporting documentation. The focus is on clearly outlining how symptoms impact daily functioning, work capacity, and overall quality of life in alignment with SSA criteria. This is a private pay service and is not billed to insurance. What’s Included: Comprehensive psychiatric evaluation (60 minutes) Review of medical and psychiatric history Functional assessment including activities of daily living and work limitations Supporting clinical summary or letter, when appropriate One minor revision if clarification is requested within 7 days Includes evaluation and completion of up to 15 pages of SSI/SSDI documentation.
